Fluoroscopic-Guided Basket Retrieval of Salivary Stones

What is this procedure?

Basket retrieval is a minimally invasive technique used to remove stones (sialoliths) from the salivary ducts. A very fine wire “basket” is inserted into the duct and guided to the stone using live X-ray imaging (fluoroscopy). The basket is opened around the stone, gently grasps it, and the stone is then withdrawn through the duct.

Why is it done?

Basket retrieval may be recommended for:

  • Stones lodged in the duct causing swelling, pain, or infection
  • Stones that cannot be flushed out with normal washout techniques

What happens during the procedure?

  • You will be lying comfortably.
  • The doctor will locate the salivary duct opening inside your mouth.
  • A fine basket device is advanced to the stone under X-ray guidance.
  • The basket is opened, the stone is captured, and the device is withdrawn.
  • You may feel mild pressure or fullness but it should not be painful.
  • The procedure usually takes 20–45 minutes depending on the stone’s size and position.

After the procedure

  • You can usually go home shortly afterwards.
  • Mild soreness or swelling of the gland may occur for 24–48 hours.
  • You may be advised to massage the gland, stay well hydrated, and use sour sweets to encourage saliva flow.
  • Normal eating and drinking can continue unless told otherwise.

Possible risks

These are uncommon:

  • Temporary discomfort or swelling

  • Inability to remove the stone (occasionally another procedure is needed)
  • Rarely, irritation, narrowing, or injury to the duct

Follow-up

You will be offered a follow up appointment to discuss post procedure symptoms and whether any further treatment is needed.
